The PAM include file pam_appl.h resides in /usr/include/pam on Mac OS X, instead
of the expected /usr/include/security, so building on Mac OS X using --with-pam
fails.

Patches to get it to work properly (tested with gcc 3.3 on Mac OS X 10.3.2)
attached.

Thanks but I actually fixed this about a week ago in the sudo cvs tree using a 
similar approach.
